FCM surfaces Health Home Plus (HH+) SMI Eligibility on a member's Overview as it's downloaded from MAPP. If the member is identified as eligible, a Status Description Code is provided to determine how the member is eligible.

- 52 - AOT - Expired < 12 months
- 54 - ACT - Discharged <12 months
- 55 - 3+ Inpt MH <12 months
- 56 - 4+ ER MH <12 months
- 57 - 3+ Inpt Med & Schiz/Bipolar Dx <12 months
- 58 - Ineffectively Engaged - No Outpt MH <12 months & 2+ Inpt MH/3+ ER MH
- 59 - State PC Discharge <12 months
- 88 - HH+ eligible CNYPC Releases <12 months
- 101 - HH+ eligible homeless w/in last 6 months and SMI w/in the past year
