Consent Date on Member Overview

When creating an Enrolled Segment for a member, Users should upload their signed consent form under the Documents tab of the member chart. This is required for Adult members, as part of ensuring a segment successfully syncs to MAPP. For Child members, the consent typically aligns with the consent date used when creating their segment in the Children's Portal.

This consent date is usually aligned with the date of the member's current enrollment period.

For example, John Smith signed a consent to enroll into Health Home on 10/10/2024. The member's segment start date would typically begin 10/1/2024. Uploading a consent document with this date for this member's record will mark their consent as completed:

Consent dates follow the member upon their initial enrollment of consecutive months--so if a member was in outreach and moved into enrollment the following month, that consent date would follow. If ever there is a gap month between segments, the system will no longer recognize the existing consent as "active" and will look for a new consent to follow the "new" segment periods.
