Foothold Care Management uses the “claim type” field to group similar kinds of claims in FCM. All claims will be labeled as one of the following types:
- HH Claim / Health Home bills. This claim is for a Health Home service. The Health Home is responsible for billing for the service.
- HH Claim / Converting CMA bills. This claim is for a Health Home service. The Care Management Agency is responsible for billing for the service.
- HH Claim / MCO bills. This claim is for a Health Home service. The associated Managed Care Organization is responsible for billing for the service.
- Health Home Fee. This claim is an administrative fee assessed by the Health Home.
- Adult Home Assessment and Management Fee. This claim is an administrative fee charged by the Health Home to Medicaid for members enrolled in an Adult Home program.
